We care for post-operative incisions, pressure injuries, leg and foot ulcers, skin tears and diabetic wounds. Each is assessed, photographed and tracked against a healing plan, with dressing choices matched to the wound stage rather than whatever happens to be in the cupboard.

When you have been discharged from a hospital such as The Alfred, Cabrini, Holmesglen Private or Sandringham Hospital, we pick up where the ward left off and keep your GP and surgeon informed throughout.

How wound care at home works

Structured, monitored and coordinated from the first visit:

Assessment
A nurse assesses the wound — type, stage, size and signs of infection — and agrees a healing plan and dressing regimen with you.

Regular dressing changes
Dressings are changed on a schedule suited to the wound, using sterile technique and the right products for each stage.

Monitoring and escalation
We track healing between visits and escalate promptly to your GP or a wound specialist if the wound stalls or shows infection.
4
Healing and prevention
As the wound closes we focus on skin integrity and prevention, so it does not recur.

What types of wounds can you manage at home?
Surgical incisions, pressure injuries, venous and arterial leg ulcers, diabetic foot wounds, skin tears and other chronic or non-healing wounds.
Can you remove sutures or staples?
Yes. Suture and staple removal is a routine part of post-operative wound care, done at the appropriate time and in line with your surgeon's instructions.
Will you keep my GP updated?
Always. We document each review, track healing over time and report to your GP, wound clinic or surgeon so care stays coordinated.
Do you provide the dressings?
We advise on and can supply appropriate dressings; some patients source specific products through their GP or pharmacy. We make it clear and simple either way.
How soon can a nurse visit after discharge?
We aim to see post-discharge patients promptly, often within a day, so there is no gap in wound care when you get home.
