How is this different from the rehab hospital?
Rehab is intensive, facility-based therapy. This is nursing support at home afterwards — monitoring your health, managing medications and reinforcing your progress so the gains hold.
Do you work with my physiotherapist?
Yes. We coordinate with your physio, occupational therapist and rehab team so what happens at home reinforces your therapy rather than working against it.
Can you help prevent another fall or setback?
A large part of the role is prevention — a home-safety review, medication safety, monitoring and confidence-building all reduce the risk of a setback.
How long does post-rehab nursing last?
As long as you need and no longer, often a few weeks, tapering as you regain independence. We review it regularly with you.
